We invite you to celebrate the memory of your loved one at Community Hospice & Palliative Care's 33rd Annual Tree of Life Celebration! Our trees will be adorned with ribbons, and this year, we will have special trees recognizing our youngest patients and children from our community. We encourage all family and friends attending our Tree of Life and Candlelight Ceremony of Remembrance to bring a photo of their loved one to display on the memory table. EARL B. HADLOW CENTER FOR CARING: Free onsite parking is available, no RSVP required, and overflow parking with free shuttle service for the event will be at Watson Commercial Realty, Inc. 4685 Sunbeam Rd # 4, Jacksonville, FL 32257

STEPHEN R. CHAPMAN FAMILY COMMUNITY CAMPUS: There will be no onsite parking. Free parking and shuttle service for the event will be at Destiny Church, 1485 US-1, St. Augustine, FL 32084.

 

Personalized Ribbon Order

The 2023 Tree of Life will feature lights and ribbons representing those who have lost loved ones in our community. When you register for the event, you will be prompted to personalize up to four (4) ribbons. We hope you will join us for this meaningful and reflective service. By paying tribute to the people who hold a special place in your life through the Tree of Life celebration, you support our mission to provide compassionate, comprehensive palliative and hospice care to patients and their families in the communities we serve.

 Our Tree of Life event is free and open to the public.
